Likely pathogenic for Cardiovascular phenotype; Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_006767.4(LZTR1):c.2326-2A>G, citing Ambry Variant Classification Scheme 2023: The c.2326-2A>G intronic variant results from an A to G substitution two nucleotides upstream from coding exon 20 in the LZTR1 gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site; however, direct evidence is insufficient at this time (Ambry internal data). The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; however, direct evidence is unavailable. The exact functional effect of the altered amino acid sequence is unknown; however, the impacted region is critical for protein function (Ambry internal data). This nucleotide position is highly conserved in available vertebrate species. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this variant is likely pathogenic for an increased risk of LZTR1-related schwannomatosis (SWN) and would be expected to cause autosomal recessive Noonan syndrome when present along with a second pathogenic variant on the other allele; however, its clinical significance for autosomal dominant Noonan syndrome is unclear.
