Likely pathogenic for Holoprosencephaly 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_007129.5(ZIC2):c.1243del (p.His415fs), citing Invitae Variant Classification Sherloc (09022015): This sequence change results in a frameshift in the ZIC2 gene (p.His415Metfs*140). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 118 amino acid(s) of the ZIC2 protein and extend the protein by 21 additional amino acid residues. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with ZIC2-related conditions. This variant results in an extension of the ZIC2 protein. Other variant(s) that result in a similarly extended protein product (p.His520Profs*35, p.Ser444Alafs*111, p.Gly498Alafs*57) have been observed in individuals with ZIC2-related disease (PMID: 19177455, 19955556, 21940735). This suggests that these extensions may be clinically significant. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
