NM_001267550.2(TTN):c.86015G>A (p.Trp28672Ter) was classified as Likely pathogenic for Dilated cardiomyopathy 1G; Autosomal recessive limb-girdle muscular dystrophy type 2J by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TTN gene (transcript NM_001267550.2) at coding-DNA position 86015, where G is replaced by A; at the protein level this means converts the codon for tryptophan at residue 28672 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: This variant is not present in population databases (gnomAD no frequency). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. This variant is located in the A band of TTN (PMID: 25589632). Truncating variants in this region are significantly overrepresented in patients affected with dilated cardiomyopathy (PMID: 25589632). Truncating variants in this region have also been reported in individuals affected with autosomal recessive centronuclear myopathy (PMID: 23975875). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. This premature translational stop signal has been observed in individual(s) with dilated cardiomyopathy (PMID: 27886618). This sequence change creates a premature translational stop signal (p.Trp28672*) in the TTN gene. While this is not anticipated to result in nonsense mediated decay, it is expected to create a truncated TTN protein.
