Uncertain significance for Cobalamin C disease — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_015506.3(MMACHC):c.393A>C (p.Gln131His), citing Invitae Variant Classification Sherloc (09022015): Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MMACHC protein function. This variant has not been reported in the literature in individuals with MMACHC-related conditions. This variant is not present in population databases (ExAC no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This sequence change replaces glutamine with histidine at codon 131 of the MMACHC protein (p.Gln131His). The glutamine residue is highly conserved and there is a small physicochemical difference between glutamine and histidine.
