Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_172364.5(CACNA2D4):c.2309G>T (p.Ser770Ile), citing Invitae Variant Classification Sherloc (09022015): This variant has not been reported in the literature in individuals with CACNA2D4-related conditions. This sequence change replaces serine with isoleucine at codon 770 of the CACNA2D4 protein (p.Ser770Ile). The serine residue is moderately conserved and there is a large physicochemical difference between serine and isoleucine. This variant is not present in population databases (ExAC no frequency). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
