Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000424.4(KRT5):c.541T>C (p.Ser181Pro), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KRT5 gene (transcript NM_000424.4) at coding-DNA position 541, where T is replaced by C; at the protein level this means replaces serine at residue 181 with proline — a missense variant. Submitter rationale: This sequence change replaces serine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 181 of the KRT5 protein (p.Ser181Pro). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with epidermolysis bullosa (PMID: 10730767, 36287101). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 14650). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on KRT5 protein function. For these reasons, this variant has been classified as Pathogenic.
