Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001165963.4(SCN1A):c.5594_5595dup (p.Asp1866fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN1A gene (transcript NM_001165963.4) at coding-DNA position 5594 through coding-DNA position 5595, duplicating 2 bases; at the protein level this means shifts the reading frame starting at aspartic acid residue 1866, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the SCN1A protein in which other variant(s) (p.Ala1919Leufs*13) have been determined to be pathogenic (Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. ClinVar contains an entry for this variant (Variation ID: 1459300). This variant has not been reported in the literature in individuals affected with SCN1A-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Asp1866Leufs*12) in the SCN1A gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 144 amino acid(s) of the SCN1A protein.
