Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000375.3(UROS):c.296T>C (p.Val99Ala), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine with alanine at codon 99 of the UROS protein (p.Val99Ala). The valine residue is highly conserved and there is a small physicochemical difference between valine and alanine. This variant is not present in population databases (ExAC no frequency). This variant has been observed in individual(s) with congenital erythropoietic porphyria (PMID: 7552139). In at least one individual the data is consistent with the variant being in trans (on the opposite chromosome) from a pathogenic variant. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. Experimental studies have shown that this variant affects UROS protein function (PMID: 7552139, 7860775, 16532394, 19099412). For these reasons, this variant has been classified as Pathogenic.
