Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000374.5(UROD):c.497T>C (p.Val166Ala), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with alanine, which is neutral and non-polar, at codon 166 of the UROD protein (p.Val166Ala). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal recessive hepatoerythropoietic porphyria (PMID: 20479301). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1455082). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt UROD protein function with a negative predictive value of 80%.
