Uncertain significance for Charcot-Marie-Tooth disease type 4 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001370298.3(FGD4):c.674G>T (p.Cys225Phe), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces cysteine with phenylalanine at codon 88 of the FGD4 protein (p.Cys88Phe). The cysteine residue is highly conserved and there is a large physicochemical difference between cysteine and phenylalanine. This variant is present in population databases (rs201660852, ExAC 0.04%). This variant has not been reported in the literature in individuals affected with FGD4-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
