Uncertain significance for Neuropathy, hereditary sensory and autonomic, type 2A; Generalized epilepsy with febrile seizures plus, type 7 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001365536.1(SCN9A):c.4567G>A (p.Val1523Ile), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN9A gene (transcript NM_001365536.1) at coding-DNA position 4567, where G is replaced by A; at the protein level this means replaces valine at residue 1523 with isoleucine — a missense variant. Submitter rationale: This variant is present in population databases (rs749548794, ExAC 0.01%). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with SCN9A-related conditions. This sequence change replaces valine with isoleucine at codon 1512 of the SCN9A protein (p.Val1512Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine.
