Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018847.4(KLHL9):c.1457A>G (p.His486Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with arginine at codon 486 of the KLHL9 protein (p.His486Arg). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and arginine. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with KLHL9-related conditions. ClinVar contains an entry for this variant (Variation ID: 1438004). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt KLHL9 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
