Uncertain significance for Myasthenic syndrome, congenital, 22 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001171613.2(PREPL):c.1256A>G (p.His419Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with arginine at codon 508 of the PREPL protein (p.His508Arg). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and arginine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. This variant has not been reported in the literature in individuals affected with PREPL-related conditions. This variant is not present in population databases (ExAC no frequency).
