Pathogenic for Cataract 18 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024513.4(FYCO1):c.528_529insGGCAAAGT (p.Thr177fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FYCO1 gene (transcript NM_024513.4) at coding-DNA position 528 through coding-DNA position 529, inserting GGCAAAGT; at the protein level this means shifts the reading frame starting at threonine residue 177, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site, but this prediction has not been confirmed by published transcriptional studies. This variant has not been reported in the literature in individuals with FYCO1-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Thr177Glyfs*10) in the FYCO1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in FYCO1 are known to be pathogenic (PMID: 21636066).
