Uncertain significance for Immunodeficiency 35 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_003331.5(TYK2):c.1274G>A (p.Arg425His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine with histidine at codon 425 of the TYK2 protein (p.Arg425His). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and histidine. This variant is present in population databases (no rsID available, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with TYK2-related conditions. ClinVar contains an entry for this variant (Variation ID: 1422131). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TYK2 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
