NM_000140.5(FECH):c.1217G>A (p.Cys406Tyr) was classified as Pathogenic by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces cysteine, which is neutral and slightly polar, with tyrosine, which is neutral and polar, at codon 406 of the FECH protein (p.Cys406Tyr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with erythropoietic protoporphyria (PMID: 10942404, 28614581, 33275677; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1409435). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt FECH protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
