NM_000257.4(MYH7):c.1268C>T (p.Ala423Val) was classified as Uncertain significance for Cardiovascular phenotype by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The p.A423V variant (also known as c.1268C>T), located in coding exon 12 of the MYH7 gene, results from a C to T substitution at nucleotide position 1268. The alanine at codon 423 is replaced by valine, an amino acid with similar properties. This alteration is located in the myosin head domain, which contains a statistically significant clustering of pathogenic missense variants (Homburger JR et al. Proc Natl Acad Sci U S A, 2016 06;113:6701-6; Walsh R et al. Genet Med, 2017 02;19:192-203; Ambry internal data). This alteration has been reported in a hypertrophic cardiomyopathy clinical genetic testing cohort; however, clinical details were limited (Walsh R et al. Genet. Med., 2017 02;19:192-203). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
