Uncertain significance for Severe neonatal-onset encephalopathy with microcephaly — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001110792.2(MECP2):c.358C>G (p.Leu120Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces leucine with valine at codon 108 of the MECP2 protein (p.Leu108Val). The leucine residue is highly conserved and there is a small physicochemical difference between leucine and valine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt MECP2 protein function.
