Uncertain significance for Mucopolysaccharidosis, MPS-IV-A — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000512.5(GALNS):c.707_708inv (p.His236Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces histidine with arginine at codon 236 of the GALNS protein (p.His236Arg). The histidine residue is highly conserved and there is a small physicochemical difference between histidine and arginine. Information on the frequency of this variant in the gnomAD database is not available, as this variant may be reported differently in the database. This variant has not been reported in the literature in individuals affected with GALNS-related conditions. ClinVar contains an entry for this variant (Variation ID: 1406013). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. This variant disrupts the p.His236 amino acid residue in GALNS. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 20574428, 34813777, 35212421). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
