Uncertain significance for Joubert syndrome 18; Orofacial-digital syndrome IV — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_015631.6(TCTN3):c.362G>C (p.Cys121Ser), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces cysteine, which is neutral and slightly polar, with serine, which is neutral and polar, at codon 121 of the TCTN3 protein (p.Cys121Ser). This variant is present in population databases (no rsID available, gnomAD 0.002%). This variant has not been reported in the literature in individuals affected with TCTN3-related conditions. ClinVar contains an entry for this variant (Variation ID: 1398260). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt TCTN3 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
