Uncertain significance for Seizures, benign familial infantile, 3; Developmental and epileptic encephalopathy, 11 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001040142.2(SCN2A):c.398T>G (p.Met133Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the SCN2A gene (transcript NM_001040142.2) at coding-DNA position 398, where T is replaced by G; at the protein level this means replaces methionine at residue 133 with arginine — a missense variant. Submitter rationale: Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. This sequence change replaces methionine with arginine at codon 133 of the SCN2A protein (p.Met133Arg). The methionine residue is weakly conserved and there is a moderate physicochemical difference between methionine and arginine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with SCN2A-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
