Uncertain significance for Familial hemophagocytic lymphohistiocytosis 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006949.4(STXBP2):c.1334G>T (p.Gly445Val), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the STXBP2 gene (transcript NM_006949.4) at coding-DNA position 1334, where G is replaced by T; at the protein level this means replaces glycine at residue 445 with valine — a missense variant. Submitter rationale: This sequence change replaces glycine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 445 of the STXBP2 protein (p.Gly445Val). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on STXBP2 protein function. ClinVar contains an entry for this variant (Variation ID: 1392634). This variant has not been reported in the literature in individuals affected with STXBP2-related conditions. This variant is not present in population databases (gnomAD no frequency).
