Uncertain significance for Neuropathy, hereditary sensory and autonomic, type 2A; Pseudohypoaldosteronism type 2C — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_018979.4(WNK1):c.4172G>A (p.Gly1391Asp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the WNK1 gene (transcript NM_018979.4) at coding-DNA position 4172, where G is replaced by A; at the protein level this means replaces glycine at residue 1391 with aspartic acid — a missense variant. Submitter rationale: This sequence change replaces glycine with aspartic acid at codon 1643 of the WNK1 protein (p.Gly1643Asp). The glycine residue is moderately conserved and there is a moderate physicochemical difference between glycine and aspartic acid. This variant is present in population databases (rs769373261, ExAC 0.001%). This variant has not been reported in the literature in individuals with WNK1-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt WNK1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
