Uncertain significance for Microcephaly, normal intelligence and immunodeficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002485.5(NBN):c.1018C>G (p.Pro340Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the NBN gene (transcript NM_002485.5) at coding-DNA position 1018, where C is replaced by G; at the protein level this means replaces proline at residue 340 with alanine — a missense variant. Submitter rationale: This sequence change replaces proline with alanine at codon 340 of the NBN protein (p.Pro340Ala). The proline residue is moderately conserved and there is a small physicochemical difference between proline and alanine. This variant is not present in population databases (ExAC no frequency). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with NBN-related conditions.
