Pathogenic for Leber congenital amaurosis 8; Retinitis pigmentosa 12 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_201253.3(CRB1):c.716G>T (p.Cys239Phe), citing Invitae Variant Classification Sherloc (09022015): For these reasons, this variant has been classified as Pathogenic. This variant disrupts the p.Cys239 amino acid residue in CRB1. Other variant(s) that disrupt this residue have been observed in individuals with CRB1-related conditions (PMID: 31106028; Invitae), which suggests that this may be a clinically significant amino acid residue. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CRB1 protein function. This missense change has been observed in individual(s) with clinical features of macular dystrophy (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces cysteine, which is neutral and slightly polar, with phenylalanine, which is neutral and non-polar, at codon 239 of the CRB1 protein (p.Cys239Phe).
