Uncertain significance for Congenital neutropenia-myelofibrosis-nephromegaly syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_007259.5(VPS45):c.115T>A (p.Tyr39Asn), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the VPS45 gene (transcript NM_007259.5) at coding-DNA position 115, where T is replaced by A; at the protein level this means replaces tyrosine at residue 39 with asparagine — a missense variant. Submitter rationale: This sequence change replaces tyrosine with asparagine at codon 39 of the VPS45 protein (p.Tyr39Asn). The tyrosine residue is moderately conserved and there is a large physicochemical difference between tyrosine and asparagine. This variant is present in population databases (rs188130682, ExAC 0.01%). This variant has not been reported in the literature in individuals affected with VPS45-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C15"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
