Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000030.3(AGXT):c.215A>T (p.Asn72Ile), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the AGXT gene (transcript NM_000030.3) at coding-DNA position 215, where A is replaced by T; at the protein level this means replaces asparagine at residue 72 with isoleucine — a missense variant. Submitter rationale: This variant is present in population databases (no rsID available, gnomAD 0.01%). This sequence change replaces asparagine, which is neutral and polar, with isoleucine, which is neutral and non-polar, at codon 72 of the AGXT protein (p.Asn72Ile). This missense change has been observed in individual(s) with primary hyperoxaluria (PMID: 27644547). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1378865). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt AGXT protein function. For these reasons, this variant has been classified as Pathogenic.
