Uncertain significance for Progressive sclerosing poliodystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002693.3(POLG):c.2071G>A (p.Val691Ile), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine with isoleucine at codon 691 of the POLG protein (p.Val691Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with POLG-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
