Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_020987.5(ANK3):c.5219C>T (p.Thr1740Met), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine with methionine at codon 1740 of the ANK3 protein (p.Thr1740Met). The threonine residue is weakly conserved and there is a moderate physicochemical difference between threonine and methionine. This variant is present in population databases (no rsID available, gnomAD 0.02%), including at least one homozygous and/or hemizygous individual. This variant has not been reported in the literature in individuals affected with ANK3-related conditions. ClinVar contains an entry for this variant (Variation ID: 1375330). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt ANK3 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
