This variant lies in the GNAO1 gene (transcript NM_020988.3) at coding-DNA position 124, where G is replaced by A; at the protein level this means replaces glycine at residue 42 with arginine — a missense variant. Submitter rationale: This sequence change replaces glycine with arginine at codon 42 of the GNAO1 protein (p.Gly42Arg). The glycine residue is highly conserved and there is a moderate physicochemical difference between glycine and arginine. This variant is not present in population databases (ExAC no frequency). This missense change has been observed in individual(s) with GNAO1-related conditions (PMID: 25590979). In at least one individual the variant was observed to be de novo. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GNAO1 protein function. For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this variant affects GNAO1 protein function (PMID: 28747448).
