Uncertain significance for Hereditary cancer-predisposing syndrome; Familial thoracic aortic aneurysm and aortic dissection — the classification assigned by Ambry Genetics to NM_005359.6(SMAD4):c.520A>G (p.Thr174Ala), citing Ambry Variant Classification Scheme 2023. This variant lies in the SMAD4 gene (transcript NM_005359.6) at coding-DNA position 520, where A is replaced by G; at the protein level this means replaces threonine at residue 174 with alanine — a missense variant. Submitter rationale: The p.T174A variant (also known as c.520A>G), located in coding exon 4 of the SMAD4 gene, results from an A to G substitution at nucleotide position 520. The threonine at codon 174 is replaced by alanine, an amino acid with similar properties. This alteration was also detected on a 25-gene panel test in a woman who was diagnosed with breast cancer before age 50 (Tung N et al. Cancer, 2015 Jan;121:25-33). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
