Uncertain significance for Congenital myasthenic syndrome 17; Sclerosteosis 2; Cenani-Lenz syndactyly syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002334.4(LRP4):c.3020C>A (p.Ala1007Asp), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the LRP4 gene (transcript NM_002334.4) at coding-DNA position 3020, where C is replaced by A; at the protein level this means replaces alanine at residue 1007 with aspartic acid — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. This variant has not been reported in the literature in individuals affected with LRP4-related conditions. This variant is not present in population databases (ExAC no frequency). This sequence change replaces alanine with aspartic acid at codon 1007 of the LRP4 protein (p.Ala1007Asp). The alanine residue is moderately conserved and there is a moderate physicochemical difference between alanine and aspartic acid.
