Pathogenic for Neutropenia, severe congenital, 1, autosomal dominant; Cyclical neutropenia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001972.4(ELANE):c.461T>G (p.Met154Arg), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces methionine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 154 of the ELANE protein (p.Met154Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of severe congenital neutropenia and/or neutropenia (PMID: 25427142; Invitae). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1372707). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ELANE protein function. For these reasons, this variant has been classified as Pathogenic.
