Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_006231.4(POLE):c.588G>C (p.Gln196His), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 196 of the POLE protein (p.Gln196His). RNA analysis indicates that this missense change induces altered splicing and likely results in the loss of 4 amino acid residue(s), but is expected to preserve the integrity of the reading-frame. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 1370165). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt POLE protein function with a negative predictive value of 80%. Studies have shown that this missense change results in the activation of a cryptic splice site in exon 7 (Invitae).
