Uncertain significance for COG4-congenital disorder of glycosylation — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_015386.3(COG4):c.1124G>A (p.Arg375His), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the COG4 gene (transcript NM_015386.3) at coding-DNA position 1124, where G is replaced by A; at the protein level this means replaces arginine at residue 375 with histidine — a missense variant. Submitter rationale: This sequence change replaces arginine with histidine at codon 375 of the COG4 protein (p.Arg375His). The arginine residue is highly conserved and there is a small physicochemical difference between arginine and histidine. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with COG4-related conditions. This variant is present in population databases (rs762087790, ExAC 0.009%).
