Uncertain significance for Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 4; Dyskeratosis congenita, autosomal recessive 6 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_002582.4(PARN):c.664C>G (p.Pro222Ala), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the PARN gene (transcript NM_002582.4) at coding-DNA position 664, where C is replaced by G; at the protein level this means replaces proline at residue 222 with alanine — a missense variant. Submitter rationale: This sequence change replaces proline with alanine at codon 222 of the PARN protein (p.Pro222Ala). The proline residue is highly conserved and there is a small physicochemical difference between proline and alanine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with PARN-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
