Uncertain significance for Retinitis pigmentosa 54 — the classification assigned by 3billion to NM_001029883.3(PCARE):c.551A>G (p.His184Arg), citing ACMG Guidelines, 2015. This variant lies in the PCARE gene (transcript NM_001029883.3) at coding-DNA position 551, where A is replaced by G; at the protein level this means replaces histidine at residue 184 with arginine — a missense variant. Submitter rationale: The variant is observed at an extremely low frequency in the gnomAD v2.1.1 dataset (total allele frequency: <0.001%). Predicted Consequence/Location: The majority of the known disease-causing variants of this gene are variants expected to result in premature termination of the protein. In silico tool predictions suggest damaging effect of the variant on gene or gene product [REVEL: 0.67 (>=0.6, sensitivity 0.68 and specificity 0.92); 3Cnet: 0.64 (>=0.6, sensitivity 0.72 and precision 0.9)]. Same nucleotide change resulting in same amino acid change has been previously reported to be associated with PCARE related disorder (ClinVar ID: VCV001363488). However, the evidence of pathogenicity is insufficient at this time. Therefore, this variant is classified as VUS according to the recommendation of ACMG/AMP guideline.
