Pathogenic for Walker-Warburg congenital muscular dystrophy — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024301.5(FKRP):c.650dup (p.Val218fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the FKRP gene (transcript NM_024301.5) at coding-DNA position 650, duplicating one base; at the protein level this means shifts the reading frame starting at valine residue 218, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: ClinVar contains an entry for this variant (Variation ID: 1363219). For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the FKRP protein in which other variant(s) (p.Trp432*) have been determined to be pathogenic (Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with FKRP-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Val218Glyfs*41) in the FKRP gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 278 amino acid(s) of the FKRP protein.
