Uncertain significance for Neuroblastoma, susceptibility to, 3 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004304.5(ALK):c.1779G>C (p.Trp593Cys), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the ALK gene (transcript NM_004304.5) at coding-DNA position 1779, where G is replaced by C; at the protein level this means replaces tryptophan at residue 593 with cysteine — a missense variant. Submitter rationale: This variant is not present in population databases (ExAC no frequency). This sequence change replaces tryptophan with cysteine at codon 593 of the ALK protein (p.Trp593Cys). The tryptophan residue is highly conserved and there is a large physicochemical difference between tryptophan and cysteine. This variant has not been reported in the literature in individuals with ALK-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
