NM_007294.4(BRCA1):c.81-2A>C was classified as Pathogenic for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at the canonical splice acceptor site of the intron immediately before coding-DNA position 81, where A is replaced by C; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.81-2A>C intronic pathogenic mutation results from an A to C substitution two nucleotides upstream from coding exon 2 in the BRCA1 gene. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and may result in the creation or strengthening of a novel splice acceptor site. The stop codon in the predicted resulting transcript occurs in the 5' end ofthe BRCA1 gene. As such, this alteration may escape nonsense-mediated mRNAdecay and/or be prone to rescue by reinitiation (Rivas et al. Science. 2015 May 8;348(6235):666-9; Lindeboom et al. Nat Genet. 2016 Oct;48(10):1112-8; Rhee et al. Sci Rep. 2017 May 10;7(1):1653). The exact functional effect of this alteration is unknown; however, the region predicted to be impacted is critical for protein function and a significant portion of the protein is affected (Ambry internal data). One functional study found that this nucleotide substitution is non-functional in a high throughput genome editing haploid cell survival assay (Findlay GM et al. Nature, 2018 Oct;562:217-222). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.
