NM_005359.5(SMAD4):c.(?_-1)_249+?dup was classified as Likely benign for Juvenile polyposis syndrome by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): Data suggest that this duplication is the result of a processed mRNA transcript that has inserted into the genome, also known as a processed pseudogene or retrotransposition event. This retrocopy insertion has been determined to be on chromosome 9 and is not expected impact the SMAD4 gene located on chromosome 18. This sequence change has been reported in the literature and is not reported in population databases. A SMAD4 retrotransposon has been reported on chromosome 9 in two samples from a control population but the exact insertion site and orientation of the retrocopy were not described (PMID: 24026178). The functional impact of a retrocopy depends on multiple factors, including the exons involved, the flanking sequence, and genomic location (PMID: 23359205, 23497673, 18842134). A series of experiments designed to determine the exact location has confirmed insertion of this SMAD4 retrocopy including exons 1-12 into the last intron of SCAI gene on chromosome 9 (Invitae data) and therefore it is not expected to interfere with SMAD4 gene function. Because this retrotransposition lies outside the SMAD4 locus, it has been classified as Likely Benign for SMAD4.
