This variant lies in the POLG gene (transcript NM_002693.3) at coding-DNA position 2869, where G is replaced by T; at the protein level this means replaces alanine at residue 957 with serine — a missense variant. Submitter rationale: This sequence change replaces alanine, which is neutral and non-polar, with serine, which is neutral and polar, at codon 957 of the POLG protein (p.Ala957Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with autosomal dominant progressive external ophthalmoplegia (PMID: 12210792). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 13508). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt POLG protein function with a positive predictive value of 95%. Experimental studies have shown that this missense change affects POLG function (PMID: 15258572, 23208208). For these reasons, this variant has been classified as Pathogenic.
