Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_144701.3(IL23R):c.1675A>C (p.Ser559Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IL23R gene (transcript NM_144701.3) at coding-DNA position 1675, where A is replaced by C; at the protein level this means replaces serine at residue 559 with arginine — a missense variant. Submitter rationale: This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 559 of the IL23R protein (p.Ser559Arg). This variant is present in population databases (rs564574754, gnomAD 0.2%), and has an allele count higher than expected for a pathogenic variant. This variant has not been reported in the literature in individuals affected with IL23R-related conditions. ClinVar contains an entry for this variant (Variation ID: 1345358). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change does not substantially affect IL23R function (PMID: 30578351). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
