Uncertain significance — the classification assigned by Genetic Services Laboratory, University of Chicago to NM_003482.4(KMT2D):c.6276C>T (p.Asp2092=), citing ACMG Guidelines, 2015. This variant lies in the KMT2D gene (transcript NM_003482.4) at coding-DNA position 6276, where C is replaced by T; at the protein level this means the protein sequence is unchanged (aspartic acid at residue 2092 retained) — a synonymous variant. Submitter rationale: DNA sequence analysis of the KMT2D gene demonstrated a sequence change, c.6276C>T, in exon 31 which does not result in an amino acid change. This sequence change does not appear to have been previously described in patients with KMT2D-related disorders and has not been described in the large population databases such as EXAC or gnomAD (dbSNP rs1388239595). This sequence change is not predicted to have a deleterious effect on splicing based on in silico splice prediction programs. As the c.6276C>T sequence change does not result in a change in the KMT2D amino acid sequence, it is possible that this change is non-pathogenic and represents a benign sequence variant of the KMT2D gene, however functional studies have not been performed to prove this conclusively. The functional significance of this sequence change is not known at present and its contribution to this patient's disease phenotype cannot definitively be determined.
