Uncertain significance — the classification assigned by Genetic Services Laboratory, University of Chicago to NM_001128840.3(CACNA1D):c.1463G>A (p.Cys488Tyr), citing ACMG Guidelines, 2015: DNA sequence analysis of the CACNA1D gene demonstrated a sequence change, c.1463G>A, in exon 10 that results in an amino acid change, p.Cys488Tyr. This sequence change does not appear to have been previously described in patients with CACNA1D-related disorders and has been described in the gnomAD database with a low population frequency of 0.0014% (dbSNP rs375512160). The p.Cys488Tyr change affects a moderately conserved amino acid residue located in a domain of the CACNA1D protein that is not known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Cys488Tyr substitution. Due to these contrasting evidences and the lack of functional studies, the clinical significance of the p.Cys488Tyr change remains unknown at this time.
