This variant lies in the TBCK gene (transcript NM_001163435.3) at coding-DNA position 2044 through coding-DNA position 2048, deleting 5 bases; at the protein level this means shifts the reading frame starting at phenylalanine residue 682, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The p.Phe682fs variant in TBCK has not been previously reported in the literature in individuals with TBCK-related intellectual disability syndrome, but has been identified in 0.002% (1/59912) of Latino/Admixed American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs557016175). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 1331665) and has been interpreted as likely pathogenic by Greenwood Genetic Center Diagnostic Laboratories. This variant is predicted to cause a frameshift, which alters the protein‚Äôs amino acid sequence beginning at position 682 and leads to a premature termination codon 6 amino acids downstream. This alteration is then predicted to lead to a truncated or absent protein. Loss of function of the TBCK gene is an established disease mechanism in autosomal recessive TBCK-related intellectual disability syndrome. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive TBCK-related intellectual disability syndrome.
