This variant lies in the HEXB gene (transcript NM_000521.4) at coding-DNA position 988, where T is replaced by G; at the protein level this means replaces tyrosine at residue 330 with aspartic acid — a missense variant. Submitter rationale: Variant summary: HEXB c.988T>G (p.Tyr330Asp) results in a non-conservative amino acid change located in the Glycoside hydrolase family 20, catalytic domain (IPR015883) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251410 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.988T>G has been reported in the literature as a compound heterozygous genotype with another pathogenic variant, c.1597C>T (p.Arg533Cys) in at-least one individual affected with an infantile form of Sandhoff Disease (Bilgner_2021). To our knowledge, no variant specific experimental evidence demonstrating an impact on protein function has been reported, although both Serum total Hexosaminidase and Serum HEXA enzyme activity levels were reduced at approximately 22-26% below the lower end of the reported reference ranges for the individual with the compound heterozygous genotype ascertained above (Bilgner_2021). No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, until additional clinical reports supported by a variant specific functional evidence is obtained, the variant was classified as uncertain significance.
