NM_002772.3(TMPRSS15):c.1350dup (p.Val451fs) was classified as Pathogenic for Enterokinase deficiency by Kasturba Medical College, Manipal, Kasturba Medical College, Manipal, Manipal Academy of Higher Education, Manipal, India, citing ACMG Guidelines, 2015. This variant lies in the TMPRSS15 gene (transcript NM_002772.3) at coding-DNA position 1350, duplicating one base; at the protein level this means shifts the reading frame starting at valine residue 451, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: A known pathogenic frameshift insertion, c.1350dup in exon 12 of TMPRSS15 was observed in homozygous state in proband (Madhusudhan et al., 2021; ClinVar ID: VCV001323696.5), which is associated with enterokinase deficiency (MIM# 226200). This variant is present in 20 individuals in heterozygous state and absent in homozygous state in the population database, gnomAD v4.1.0. The variant is absent from our in-house database of 4255 exomes. The variant is predicted to cause a shift in the reading frame of the transcript introducing a premature termination codon which will either lead to the formation of a truncated protein product or nonsense-mediated mRNA decay.
