Uncertain significance — the classification assigned by Dubai Health Genomic Medicine Center, Dubai Health to NM_006908.5(RAC1):c.23_35+12dup, citing ACMG Guidelines, 2015: The c.23_35+12dup variant in RAC1 has not been previously reported in affected individuals and was absent from large population studies such as the Genome Aggregation Database (gnomAD) and the Greater Middle East (GME) Variome Database. This variant results in the introduction of 25 extra nucleotides at the splice junction of exon 1 which is then predict to impair splicing and lead to abnormal protein product. However this information is not predictive enough to confirm pathogenicity. In summary additional information is needed to fully assess the clinical significance of this variant. RAC1 is reported to play a role in coordinating immune defenses and chronic inflammatory bowel diseases are associated with dysregulation of immune defenses. Increased expression of RAC1 was associated with susceptibility to colitis by an increased inflammatory response (PMID: 21684284 PMID: 24931457). First-line immunosuppressive treatment for IBD includes azathioprine and its immunosuppressive effect is primarily ascribed to Rac1 suppression (PMID: 27604084).
