Likely Pathogenic for Nemaline myopathy — the classification assigned by Broad Center for Mendelian Genomics, Broad Institute of MIT and Harvard to NM_001164508.2(NEB):c.1266T>A (p.Tyr422Ter), citing ACMG Guidelines, 2015. This variant lies in the NEB gene (transcript NM_001164508.2) at coding-DNA position 1266, where T is replaced by A; at the protein level this means converts the codon for tyrosine at residue 422 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The p.Tyr422Ter variant in NEB has not been previously reported in the literature in individuals with nemaline myopathy, but has been identified in 0.0002% (2/1179354) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP ID: rs2149360884). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 1285142) and has been interpreted as pathogenic by Genome Diagnostics Laboratory (University Medical Center Utrecht and Joint Genome Diagnostic Labs from Nijmegen and Maastricht, Radboudumc and MUMC+). This nonsense variant leads to a premature termination codon at position 422, which is predicted to lead to a truncated or absent protein. Loss of function of the NEB gene is an established disease mechanism in autosomal recessive nemaline myopathy. In summary, although additional studies are required to fully establish its clinical significance, this variant is likely pathogenic for autosomal recessive nemaline myopathy.
